If you’re coming to our orthodontic office for the first time and are unfamiliar with our facilities and team, we want to help make the process more seamless and stress-free for you. That’s why we’re providing you with all of the information you need beforehand, allowing us to jump right in and provide you with smooth and efficient care when you arrive. Your First Visit
When you first arrive to our orthodontic office, you’ll notice your name on our welcome board as you walk in. Before meeting with Dr. Jones and Dr. Hanson, our team will take digital X-rays and all necessary scans of your mouth so we can conduct a thorough assessment of any underlying issues. Once we look over all of the images and information we’ve collected, you and Dr. Jones or Dr. Hanson will discuss treatment options. If you have any financial concerns having to do with insurance or additional flexible payment solutions, our team will gladly help out! Once you decide to move forward with your treatment, we’ll go ahead and get the ball rolling by scheduling your next appointment or even starting that very same day.
New Patient Forms
It’s understandable that you’re busy and have more places to get to other than our orthodontic office, which is why we try to make your visit as convenient and short as possible. For your initial appointment, we’ll need to collect your insurance information, dental and medical history, and other details when you arrive. By completing these forms ahead of time, we’ll be able to process this paperwork faster and bring you back to the treatment room right away.

Dental insurance coverage for orthodontic care may not be in your plan. In these cases, we also offer easy, flexible in-house financing that you can apply for. That way, you don’t have to break your monthly budget in order to start treatment. Instead of paying everything up-front, we’re able to break up the cost into small, easily manageable monthly payments.
